Quick answer: To fill in a French form, write your last name (nom) in capitals, your first name (prénom), your birth date as day/month/year, your address with number, street, postal code and city, and tick your family status (situation familiale). For French A1 students practising French forms vocabulary, this worksheet gives 20 problems from easy to hard: nom or prénom?, match form words and read the date. This worksheet trains the writing skills a newcomer needs for everyday paperwork at CEFR A1: naming the fields of a form, writing dates and addresses in the French order, and giving family status with the right agreement. It follows the A1 writing standard.
Problems 1 to 7 are guided, 8 to 15 ask for short answers and small readings, and 16 to 20 ask for texts of your own.
